Getting your kids on TOA is easy...

There are 4 Steps to enroll:

  • Section 1 - Parent/Caregiver Information

  • Section 2 - Childs Information (enroll up to 5 Children)

  • Section 3 - Alternative/Emergency contacts and pick up permissions

  • Section 4 - Payment, Permissions and Terms and Conditions

⚠️ Please Note:  This is an enrolment form only. To make a booking for After School or Holiday Care please contact the TOA office.

Section 3 - Additional & Emergency Contacts

Please provide at least two additional contacts who can be contacted in case of an emergency and who you will allow to drop off and pick up your children. Please note alternative contacts must be over the age of 18 and be able to pick up children if the parent or primary cannot.  

Section 4 - Payment and Permissions
